Re "lit. the gall has swollen"
膽 means gallbladder rather than its contents (gall). (It's used figuratively here, of course -- meaning "courage").
肥 is more like "ample" than "swollen".

"It means you are getting so bold that you start doing something you shouldn't do or you have no guts to do before. Usually it is used in a humorous way. For example, if someone had a fight with his boss, you can say "你胆儿肥了呀你" to banter with him.
