1. MoE says "collar bone". Others "scapula". Web discussions seem to indicate that there is strong evidence for the latter.

2. Given that the meaning does seem unknown today, and it appears mainly in historical novels, I think it can be labeled /old/

Editor: Most dictionaries have the scapula sense.
NA and NC label it [方], which is not inconsistent with (old), I think.
