1) It's a *predicate* (essentially functioning as an intransitive verb) -- not a complete sentence ("Despite superficially new policies, the substance remains unchanged.")

2) I don't think the lit. part is helpful.

Also, I think it's wrong -- "adjust one's head"?
M says 本指一代新人換舊人,面孔不斷變換。 Nothing to do with "adjusting" one's own head", whatever that's supposed to mean.

3) As LA points out, it has a non-derogatory meaning in Taiwan.
(臺) 比喻澈底改變。例 「經過父親的一番教誨,他決定改頭換面,重新做人」。
Indeed, almost every one of the many examples in TP have no derogatory tone -- see some of the examples below.
You can see derogatory usage in J, however.
