I believe there should be a single apostrophe in between touching syllable boundaries when two vowels meet?

Editor:
1) Pinyin with tone marks:
Apostrophes are used to indicate syllable boundaries.

The rule is:
"Put an apostrophe before any syllable that begins with a, e, or o, unless that syllable comes at the beginning of a word or immediately follows a hyphen or other dash."

2) Pinyin with tone numbers:
Syllable boundaries are already clear, so apostrophes aren't needed.
Diff:
# - 掏耳 掏耳 [[tao1er3]] /to remove earwax with an ear pick/
# + 掏耳 掏耳 [[tao1'er3]] /to remove earwax with an ear pick/
