每分每秒 is, at least in typical usage (see examples below), is a noun phrase, whereas "all the time" is used adverbially, as in
- "the airfield was in use all the time"
- "Oh, this happens all the time, don't worry."

每分每秒 is often used to convey the intensity with which time is experienced or utilized, whereas "all the time" is commonly used to indicate a high frequency of occurrence.

Ex.
被貶至海南島的文人,自有另一番折磨。「只有在天涯海角,絕壁死谷,生命被逼到了最後的邊界,一切才變得深刻。」(頁二二一)在順境中的每一顆生命,時光流逝並不具特別意義;而在苦難中的生命,每分每秒都必須仔細度過,存在的意義就在這樣的時光裡顯現出來。
