回天乏术 is not about an *individual's* inability to do sth.
It's not about personal failure.

Rather, it means there is nothing that *anyone* can do.

前台大醫院精神科醫師林憲,在《心路與心病》書中描述一位精神病患的自殺方式,令人印象深刻。這位住院病人在護士打破牛奶瓶時偷偷撿了一塊碎片,然後面無表情地在被中割破腹部,直到護士發現病人臉色蒼白時,已經回天乏術。
Lin Hsien, psychiatrist formerly at NTU Hospital, gives a disturbing description of the suicide of one psychiatric patient in his book The Mind and Mental Illness. When a nurse dropped a milk bottle the patient picked up a fragment of glass unnoticed, and calmly used it to slit his own belly under the covers. By the time that the nurses saw him turning pale he was already beyond saving.

Tsai rang the bell for the doctor, but CCK was already beyond help. He died just like that, in one of his bodyguard's arms.

然而,初期的人體實驗進行得並不順利。由於實驗對象是非常脆弱的小嬰兒,一個小感冒、或因食道肌肉損壞導致吞嚥困難、喝牛奶嗆到等等,就會引發肺炎,甚至造成意外死亡。有些案例則因給藥太晚,身體肌肉組織已經產生不能恢復的損壞,即使給予治療仍舊回天乏術。
However, the initial human trials did not go smoothly. Because the infants in the trials were extremely weak, a minor cold or damage to esophageal muscle leading to difficulty in swallowing could cause the child to choke on milk, leading to pneumonia or even death. In some cases, treatment proved futile because the medication was administered too late, and the body's muscular tissue had already been irreversibly damaged.

一年間她自殺3次,前2次因為「經驗不足」和家人發現得早,幸運搶救回來;第3次被發現時幾乎已回天乏術,重度昏迷期間她經歷了一段扭轉生命的瀕死體驗,並奇蹟似地生還。
She made three attempts to end her life over the course of just one year. She was quickly discovered and revived by family members the first two times, but came very close to succeeding on her third attempt. In fact, she was comatose and almost beyond help when she was found. She says it was a life-changing vision that drew her back from death's door and allowed her nearly miraculous recovery.
