"重口味" refers to food with strong flavors itself. (meanwhile the reverse, "口味重" refers to someone who likes food with strong flavors.)

This is the primary meaning, hence first position. "hardcore" etc. are derived meanings.

1) Your 3rd and 4th examples do not contain the expression 重口味.


2) 重口味 is not food, so it doesn't mean "strongly flavored food". It's an *attribute* of food.


3) Regarding each of the three senses I have added:

- 重口味乳酪 sharp cheeses (your example)

(This is when 口味 means "taste or preference (in food)" as opposed to "flavor".)
