The examples below show that the outcome is not necessarily sth unfortunate (mishap, accident etc). In fact, it's quite often sth a very *fortunate* outcome. In the 10 examples below, it's an unhappy outcome in only about 3 cases. So "accident" isn't really appropriate in our def. The examples also show that - the causes are not necessarily "many" - {"freak" combination} is a bit too strong - it's usually adverbial or phrasal, rather than purely a noun. See the examples. Sometimes it's explicitly adverbial, as in two of the sentences below where it's followed by 地.
