turning the word around in my head I can think up different uses of the word, and by looking specifically for them (adding more words in the search for context) I can find examples on the web

1. losing proper tuning, losing accuracy
(forecast)
每個人都會有投籃失準的時候 (shooting at the hoop -- somewhat more exotic -- I think it's better thought of as "off-kilter" rather than "subpar")

2. losing proper pitch (of course conflates with #1)
(guitar)

3.
GPS (failed because of GPS)
俄羅斯將手機基地台納入飛彈防禦系統,將以訊號干擾使飛彈失準 (they want to perturb the rocket's trajectory obviously)
--------------------------

Editor: I hope this is what you meant (or is at least acceptable).
Diff:
- 失準 失准 [shi1 zhun3] /not up to scratch/subpar/
# + 失準 失准 [shi1 zhun3] /not up to scratch/subpar/(of an instrument, a forecast etc) to lose accuracy/to have lost tuning/(of a musical instrument) to go off-key/(of a rocket etc) to go off-trajectory/to fail/
+ 失準 失准 [shi1 zhun3] /not up to scratch/subpar/off/gone awry/(of an instrument) to be out of kilter/(of a forecast) to be off the mark/
