地步 is punctual, not exactly "extent"

究竟要做多久才能达到女人感受到快乐的地步呢?
the plight of those poor women... or is it the man that suffers?

Editor: Our definition appears to come from both ACE (which has "plight") and later, via Mr. K, A (which doesn't have "plight", but otherwise more or less copied ACE).
Of course, ACE provided five usage examples which made the glosses somewhat clearer than they are in A, which has just one example for three senses.
