Both ambiguous and off the mark, I'd say Some other dicts have things like "in a terrible fix" (that goes back to ACE), but that tends to suggest that one is at a loss what to do, whereas in TP usage, 焦頭爛額 usually means you *know* what you have to do, but you just have difficulty managing to do it. "hard pressed" Yes! I considered leaving out the lit. part altogether, but M's explanation seems to suggest where the idea of "under pressure" came from (i.e. from trying frantically to put a fire out).
