1) This is the usual character used for "kame" (tortoise; turtle) in Japan
2) It doesn't seem to be used much in China -- not in M, TP, for example -- Z says 古同“龟”
3) It's not necessary to give the meaning in this case, since the variant's meaning is the same as the main character's meaning

There doesn't seem to be any "visible rule" in this instance.
(although you might argue that, since it's an "old" variant (Z), it *should* be on the (incomplete) list of deprecated variants in 异体字整理表)
