The second ma is also in third, not in neutral tone. Cf entry in 现代汉语词典, p. 908.

Hmm, that sounds very authoritative!

Well, it's touching that you have so much faith in 现代汉语词典 :-)
But I'm not so sure that I believe that it's always [ma3].

Yes, it's [ma3] in ~
- nciku
- Dr.eye
- Oxford Concise
- 现代汉语词典,

but it's [ma5] at ~
- ABC
- New Century
- New Age
- Far East
- MoE (dict.revised.moe.edu.tw)

You know what? I think the pronunciation may vary from person to person!
(just like "often" in English -- with or without the "t")

Of course, [ma3], coming before a 1st tone, would be subject to tone sandhi and would be pronounced as a half-3rd tone (a "fall from half-low to low" [YR Chao]), which could be a bit hard to distinguish from [ma5], when spoken quickly.
Diff:
# - 馬馬虎虎 马马虎虎 [ma3 ma5 hu1 hu1] /careless/casual/vague/not so bad/so-so/tolerable/fair/
# + 馬馬虎虎 马马虎虎 [ma3 ma3 hu1 hu1] /careless/casual/vague/not so bad/so-so/tolerable/fair/
