The principle is not to use context tags except when necessary (e.g. for disambiguation, or to identify the context in the case of a term rarely used by a non-specialist, such as "cholelithiasis"). There's no other sense of "gallstone" to distinguish from, and "gallstone" is commonly used by non-specialists.
