The encyclopedia says that ma shi qian ti (literally 'the front hoofs of the horse failing' (see photo)) is a metaphor for the occurence of 差错: error (in data transmission) / mistake / slip-up / fault / error in data transmission / accident / mishap; and 受挫: thwarted / obstructed.

I have suggested putting in /accident/mishap/mistake/obstruction/
