Problem #1
Our glosses
- "it is for...to"
and
- "to leave it (to sb)"
seem to be the same or closely related senses, yet they are separated in our definition.
It seems to be the same as this sense in ABC:
②be up to sb.; rest with sb. 運輸問題由他們解決。 The transportation issue is to be resolved by them.

I suggest removing "to leave it (to sb)"

The gloss "to" (after "due to") seems wrong.
Can anyone see how 由 can mean "to" sometimes?
Sure, it can mean "from".
But "to" as well?

I don't think 由 (on its own, not in a compound) is a noun in modern spoken Mandarin, let alone a noun meaning "reason" or "cause".
Notice that K doesn't include those senses.

"by (introduces passive verb)"
I don't think that's right.
The pattern is "由 N V" and the verb isn't a passive verb in that expression -- except when translated into English: "to be V'ed by N".
I think "by, via, through" expresses the meaning better (as in K).
Diff:
- 由 由 [you2] /to follow/from/it is for...to/reason/cause/because of/due to/to/to leave it (to sb)/by (introduces passive verb)/
+ 由 由 [you2] /to follow/from/because of/due to/by/via/through/(before a noun and a verb) it is for ... to .../
